I am trying to renew my passport that expired 2 years ago.
I have completed the online application form.
2 sections are blank on the application. 1. name appearing on birth certificate and citizenship.
is this acceptable to complete the application.

also, can i mail the application, copes of certified ID and passport photos to the consulate? or am I required to deliver them in person?
I live in Villa Ricca so attending in person will take me a two day round trip. I will copy and certify the expired australian passport as per requirements.
