- Driving license, student card or residence card
- Two color passport photos on a white background
- An extract or original marriage certificate if the applicant is a married woman who wants to change her name
- Original divorce certificate if the applicant is divorced and wants to change her name
- Parental authorization signed by the parent or legal guardian (on behalf of a minor)
- Husband’s death certificate if the applicant is widowed and wants to change her name
