Types of Passport

Procedure to apply and obtain Poland Passport

Complete the application form with accurate and up-to-date information.
Be sure to follow the provided instructions and guidelines.
Locate the nearest Passport Offices
Obtain the application form in person or download it from the official website if available.
Pay the applicable passport fees.
Submit the completed application form and all the required documents and fees to the passport office.
An employee will verify the supporting documents provided. If the request is rejected, the concerned person will be informed through the passport office with a personal letter.
Wait for the processing of your passport application. This can take some time, so it’s advisable to apply well before any planned travel.
Once your passport is ready, you will be notified to collect it from the passport office.
Some countries may require applicants to provide biometric data such as fingerprints or facial recognition. Be prepared to comply with any such requirements.

Adults Passport

Current Color photograph
Payment of Passport Proof
Document confirming fee waiver (if you are entitled to a fee waiver or exemption)
Copy of Polish marriage certificate (if your surname has changed after marrying abroad)
Polish civil status certificate copy

Child Passport

Completed Application form

Current Color photograph
Nationwide Large Family Card
A document stating the scope of parental authority (if parental rights have been lost or limited)

Adults under 70	140 PLN
Child above 12, adult with disabilities, students between 18 and 26 years old and few more groups	70 PLN
Students to 25, with valid Large Family Card	35 PLN
Adults over 70	Free
Child under 12	30 PLN
Child under 12 with valid Large Family Card	15 PLN
